Option 1: On-site screening Schedule your appointment > When scheduling your appointment, be sure to enter your legal name as it is in the county’s VCHRP system, including surname suffixes, i.e. Jr, Sr, III, etc.
The purpose of the on-site screening is to help you "know your numbers" by providing you with an annual update of your biometric results: cholesterol, HDL, LDL, triglycerides, glucose, weight and blood pressure. Once done with the screening, participants have the option to discuss their results.
To ensure your privacy, the County has contracted with Onsite Wellness, a third party vendor to conduct biometric screenings at the Hall of Administration and other designated county worksites. Your individual screening results will be provided directly to you by an Onsite Wellness technician, and will not be shared with any County employee (only participants’ aggregate data will be shared with the County to evaluate program effectiveness).

Disabling your pop-up blocker > After getting your biometric results, the next step of the Wellness Profile is to complete the online questionnaire.*
This series of questions, combined with the addition of your most recent biometric results, will provide you with a robust report of your health status so you can take action to address any health risks or issues you may have. The biometric results are entered toward the end of the questionnaire in Section 12. Although, if you leave the biometric section blank, you will still receive a report, but it won't be as accurate as if you entered your actual results.
Option 2: Personal doctor physical exam
If you can't make a county onsite screening you have the option of getting your biometric screening through an annual physical with your doctor. Below is the link to the submittal form to certify that you completed your annual physical. Also included is the program acknowledgment form; both forms need to be signed and returned to VC-WELL.
